SCH PPO and HMO insurance reminder

PPO plan reminder

Employees with the SCH PPO plan who use SCH for services will pay $0 deductible at SCH beginning in 2010. The $0 deductible is applied for services that are provided and billed by SCH. A deductible is the amount of money an employee pays before the coinsurance (percentage) kicks in.

Physician services are billed outside of SCH, and will continue to collect deductibles, copayments and coinsurance in the same manner as they have in the past. A copayment is a dollar amount paid at the time of service. Physician office copayments are $20, Emergency Room copayments are $75. These amounts are collected at the time of service.

A co-insurance is a specified percentage (at SCH it is 10%, in-network outside of SCH it is 30%) of covered medical expenses that an employee pays for services under a medical plan. SCH discounts the coinsurance after the claim is processed by the insurance company.

When you use SCH for services, please always identify yourself as an employee when registering to make sure that your employee discounts are applied to your bill.

Employee HMO Plan Reminder

HMO plan has always had a $0 deductible at SCH. A deductible is the amount of money an employee pays before the coinsurance (percentage) kicks in. The HMO Plan covers all services at 100% coinsurance. There is no additional cost to employees after the copayment. A copayment is a dollar amount that is paid at the time of service. Copayments in the HMO plan are $20 at Primary Care Physician office $40 at Specialist. Emergency Room copayments are $75.
