“Pass the Beaker” Submission Form

After you fill out the information boxes below and click "Submit," your photo will appear in the Photo Gallery and you will be eligible to win a prize in our monthly raffle.


Contest details

Please take a photo that best portrays how you contribute to the Science of Feeling Better or how you are helping meet our $3 million financial goal.

The catch: The photo must include the "Beaker," which the Marketing Department will pass to all SCH departments and nursing units, one at a time, throughout the next year. This oversized beaker represents our financial campaign efforts, as well as science.

When your department receives the beaker, you will have several days to take photos with it.

This PDF lists your department name and the dates you will have the beaker for photos.

If you would like to take photos of the beaker before your allotted time frame, or if you will not be available during that time, you can e-mail Marketing at wellinformed@swedishcovenant.org and request a new date.

How to submit

  1. Marketing will “pass the beaker” to each department for a certain length of time, depending on the size of your department.
  2. Before the beaker reaches your department, brainstorm ideas with your colleagues.
  3. When you have the beaker, take photos.
  4. Once you have a photo or two that you like, click “Show us” on this page and browse for your photo(s) on your computer.
  5. In the “Tell us” box, in 25 words or less, explain what is happening in the photo and why you believe it represents science or money saving efforts.
  6. Fill out the “About you” information boxes, as well as the encoded security letters.
  7. Click “Submit”
  8. Wait to hear from Marketing about the contest winners each month.


Rules

  • You may submit as many photos as you would like, but you can only win a prize one time
  • Photos must be taken at work
  • Do not take photos of patients or visitors
  • We prefer you upload photos from your home computer
  • This photo gallery will run through February 28, 2010.
